The required LHSAA forms (blank) are in the forms section. These forms must be completed annually prior to all student athletes participation in fall practice or games. If you have not filled these forms out please print them and do so. Have your son bring the completed form into the Coach so that we may place them on file for LHSAA inspection. If you are not sure if you have completed the required forms, please call the School Office or the Football Office to make sure.
